• To exchange bonuses for additional minutes you are required:

• 1. Print out a sample form of the Additional Agreement.
• 2. Fill in two copies of the Additional Agreement:
• Select the number of the phone you are willing to use to get bonus minutes, indicate it in the first sentence of the Additional Agreement (please note that this phone number should not be subscribed to the Additional Agreement);
• Indicate the amount of bonus minutes you are willing to use monthly, in Section 1.2. herein;
• If you wish to change your service plan simultaneously, mark the service selected for transfer in the Section 1.7 herein;
• Write down your last name, serial and number of your passport and affix your signature (legal entities are required to affix a stamp and the signature of authorized representatives) at the bottom of the page.
• 3. Mail the completed form of the Additional Agreement at the following address: UMC Call Center, CJSC "Ukrainian Mobile Communication", 15 Leiptsyzka Street, Kyiv 01015, Ukraine.

• 12-month contract term;
• 18-month contract term;
• 24-month contract term.
